First things first: you’ll need a workgroup. Transitioning to a new HMIS can feel like a lot, but a committed group leading the way will get you where you need to go! Let's go!
Actions
- Identify transition team members
- Determine roles and responsibilities for each team member
- Determine workgroup meeting structure, including attendees, frequency, and format
- Schedule your first meeting
Your work group consists of the staff who will make decisions and manage different parts of your onboarding process -- communication, training, system configuration, and more.
- Choose your team. Use our Roles and Responsibilities document to help by assigning staff to onboarding roles. Staff may perform more than one role and that’s okay. Just know that staff capacity impacts the onboarding timeline, so ensure your team has the necessary resources.
- Determine your meeting structure. How often will you meet? Does your entire workgroup need to attend every meeting? Are there others who should attend who aren’t part of your workgroup?

Actions
- Determine processes for documenting decisions
- Identify decision-makers and their domains
There are many decisions to make during onboarding. Determining your decision-makers and decision-making procedures early on ensures that decisions are made efficiently.
- Identify decision domains and assign them to staff. Use our Roles & Responsibilities and Stakeholder Roster to help. Remember that diverse perspectives support decisions that account for the needs of the broadest range of community stakeholders.
- Use our Decision Log to document decisions and reasoning. Once you’ve defined your process, begin documenting all decisions so that your team has a reference to refer to and so that future stakeholders understand the reasoning that brought about the current state.
